Welcome to Careage Place
Our admissions process is made quite simple. Upon receiving a referral, a member of our admissions team will conduct an on-site assessment. Once a determination has been made about the appropriateness of the admission, we will promptly discuss all the options with the patient, family, and referral source. Our business office will discuss issues of reimbursement such as Medicaid, Medicare, and commercial insurance.

We are located in Pilot Point, Texas.

We recognize that each resident is a unique individual with their own wants, needs, beliefs, and expectations. Consideration is given to each resident's physical medical, social and spiritual needs when designing their care program.
We know that being in a nursing facility affects more people than just the resident. Therefore, we strive to provide a home-like environment and many activities for all residents, family members, and loved ones.

Admission is made quite simple for the family as well as other referring individuals. When a call is placed to us about a patient, an on-site evaluation will usually follow to assure an appropriate placement. Medicare, Medicaid, and insurance policies can be quite confusing, our admissions team can offer assistance in determining financial resources and entitlements that may be available.

Skilled Nursing Unit

Careage Place of Pilot Point offers a specialized medical unit known as a Skilled Nursing Unit. The unit is a place for a resident to receive individual attention and care from a qualified nursing staff. The care in the Skilled Unit is always focused on the needs of the resident. Treatment and therapies are directed by a written plan of care structured specifically to the needs of each individual and ordered by your doctor. The staff focuses on rehabilitation and therapies of all disciplines physical, occupational and speech.

As a bridge between the acute care hospital and home care, a Skilled Nursing Unit can be an alternative for a seriously ill person who still requires highly skilled nursing care.
Our staff is committed to seeing every resident as an individual who needs time and therapy to assess his or her illness in the recovery process. Families are an important part of the resident's care program. The families are involved and confident in the resident's care, which increases the resident's own confidence and the chances of his or her own recovery.

Medicare Program

Careage Place of Pilot Point is a specially qualified facility that specializes in skilled care. We offer staff and equipment to provide skilled nursing care or skilled rehabilitation services.

Eligibility for Medicare is determined and established by the Social Security Administration. To be eligible for Medicare benefits, a person must meet the following requirements: age 65 or older; under age 65 with permanent kidney failure; or under age 65 and disabled.

The Medicare skilled nursing benefit is intended to provide up to 100 days of skilled nursing and rehabilitation services to residents who have been hospitalized for at least a three day period. These services were developed to assure the provision of quality care to residents and a continuation of treatment that was started in the hospital setting.

Medicaid Eligibility

If you have an established medical need for nursing home care and a financial need for assistance, the Texas Department of Human Services, Medicaid pay system will assist in paying for your nursing home care.

If you feel you need these services, please contact our business office for detailed information.

Long-Term Behavior
Management

During each generation of our life, we face specific challenges that can affect our physical and mental health. For older adults, these challenges may include declining physical or mental capabilities, the loss of lifetime companions, or the loss of independence. These changes may often cause confusion, depression, mood swings, and withdrawal. According to the National Institute of Mental Health, 18% to 25% older persons experience significant behavior problems that need professional help.

Careage Place of Pilot Point has developed an innovative program especially designed to manage the behavioral problems of older adults. We have comprised a team of caring professionals who specialize in the area of geriatrics and genuinely enjoy working with seniors. The facility is designed to cater to the needs of the behavioral resident.
